In each case you had to abandon the level and all weapons and loot. Since killing all the enemies was necessary, that meant the level couldn’t be completed. In another, an enemy ended up on the wrong side of a barrier inside a facility you were supposed to “sabotage,” and couldn’t be killed. In one case, the player character ended up *inside* a wagon.
